Winter Plan Results

KVM has concluded their Winter Plans project recently, identifying and preparing plans for dozens of their vulnerable patients that may be at risk this Winter of contracting an illness.

To reiterate, the aims of the project (supported by our local NW PHN) were to: 

  1. keep people well and out of hospital by increasing uptake of vaccinations
  2. develop and implement winter plans for preventing ill-health
  3. proactively manage deterioration of chronic or seasonal illness.

Over 160 patients were recalled, extracting information from practice management software (PMS) database on Chronic Heart Disease (CHD), Diabetes and Asthma.

139 patients were vaccinated for Influenza and COVID - 19, and 13 patients vaccinated against Shingles, Whooping Cough and Pneumococcal. 110 patients were assessed as being eligible for anti-viral medication(s).
